Opt-Out Login
If you are a member of the Class as defined in the Settlement Agreement, and you wish to exclude yourself from the Class, then you need to complete a Request for Exclusion. If you file, do not file any other form. Do not file Request for Exclusion if you wish to participate in the Settlement of this Action, including object to the Settlement. Only Class Members are permitted to object to the Settlement, and they should do so by filing an objection as described in the Notice and FAQs. If you make conflicting requests, such as by submitting both an Opt-Out Form and an objection or a request to receive money from the Settlement, your request will not be treated as a valid opt-out.
Your Opt-Out Form must be postmarked no later than December 19, 2025, or submitted electronically on or before 11:59 PM Eastern Time on December 19, 2025. If it is submitted electronically or postmarked after these dates, it will not be valid, and you will not be excluded from the Class or the lawsuit on that basis.
To start your online Exclusion Request, you must login with your Notice ID and PIN. You can find your Notice ID and PIN on the Notice you received. Please note, if you had more than one claim with Anthem, you will have multiple Notice IDs. You do NOT need to submit a Request for Exclusion for each ID, you only need to submit one.If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailinginfo@Anthem-RTC-Criteria-Settlement.com or calling (844) 496-0857. You can change your mind after you exclude yourself. If this happens, you must notify the Settlement Administrator no later than January 21, 2026. This is called a “Notice of Retraction.” This notice must: (1) be in writing; and (2) state that you changed your mind on your own, not because someone forced you. If you retract in time, you will be part of the Settlement again. This means you will get Settlement benefits and be bound by the Settlement.
